• An agreement is required to be signed during the first visit and a valid credit card is to be kept securely on file.
• A minimum 12-month commitment is required.
• Failure to complete the twelve (12) month period will result in a charge (equal to the discounts received during program) applied to the card on file or deducted from Beauty Bank at Patrick L. Basile, M.D. Plastic Surgery & Wellness.
• A $100 cancellation fee will apply if program is canceled early unless proof is provided of move 50+ miles outside of the zip code where the office is located or medical proof of illness preventing the use of the program.
• 30-Day Notice to Cancel required. All cancellation requests must be submitted in writing to 3316 3rd St South Jacksonville Beach, FL 32250 or email at orders@patrickbasilemd.com and will become effective thirty (30) days after the cancellation request is received.
• All cancellation requests will be replied to with a confirmation response.
• Loyalty Program patients must contact the office to ensure the cancellation request is being processed if a confirmation response was not received.
• Any payments due under this agreement prior to the cancellation effective date will be charged as scheduled and posted to the Loyalty Program patient's account in the Beauty Bank.
• Any unused payments will remain in the program participant's account for one (1) year after the cancellation effective date as a credit to be used for treatments or products. No monetary refunds will be given.
• Loyalty Program Beauty Bank and discounts are not transferable or combinable with other promotions, vouchers, offers, discounts and/or gift card purchases unless specified by us at the time of sale.
• We reserve the right to deny participation in our Loyalty Program to anyone at our discretion.
• We reserve the right to change these terms and conditions at any time without prior notice. In the event that any changes are made, the revised terms and conditions shall be posted immediately.
• We reserve the right to make pricing adjustments at any time without prior notice.
• Skincare & Wellness Product program discounts will not apply to ZO Products routed through the ZO website. ZO products must be purchased in office to receive discounts.
• Loyalty Program discounts apply to treatments and procedures with our Nurse Practitioners, Registered Nurses, Medical Aestheticians and CoolSculpting Specialists.
• Loyalty Program Beauty Bank and discounts are not applicable to surgery or any treatments or procedures with Dr. Basile.
• All included treatments are rendered in-office.
• Appointments are required. Appointments canceled with less than 48 hours' notice, no-shows, or same-day rescheduling are subject to a fee charged to the card on file.
• No refunds/exchanges on treatments, products, or Beauty Bank. All sales final.
